Liability Waiver:

I, the undersigned, being the parent or legal guardian of the student(s) named above understand that DANCE EXPRESS INC. and the instructors thereof are not liable for personal injuries or loss of, or damage to, personal property. I understand that participating in dance classes involves the possibility of physical injury. I therefore assume all risks involved in the participation of the DANCE EXPRESS INC. classes or workshops, whether it is I, or my participating minor child. I exempt, release and indemnify DANCE EXPRESS INC. and the faculty thereof, including instructors, volunteers and administrative assistants from any and all liability claims, demands or causes of action whatsoever from any damage, loss or physical injury to my participating minor child. By clicking the “agree” check box and typing my name below, I am acknowledging that I have read and agree to this statement and the rules and regulations set by DANCE EXPRESS INC.

Dance Express Photo Release:

I hereby grant the Dance Express Inc. permission to use my child’s likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of the Dance Express Inc. and will not be returned. I hereby irrevocably authorize the Dance Express Inc. to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge Dance Express Inc. from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Cancellation Policy:

Please be advised that class cancellations made up to October 24th, 2024 will be processed without a penalty. Cancellations made on or after October 25th, 2024 will be subject to a cancellation fee of $100.00 plus gst. Cancellations made on or after January 25th, 2025 will be subject to a $175 plus gst cancellation fee.

Registration Fees and Tuition Fees:

I understand that I will be charged a $60 registration fee (per student), this includes streaming access to my dancer’s year end performances. Tuition fees for September are due August 25th 2024.
